Home Health Care

FMH Home Health services are provided to residents of Frederick County and to the patients in our neighborhood counties: Montgomery, Carroll, and Washington as referred to us by the FMHS.

FMH Home Health Care is a wonderful place to work, and is made up of a multitude of specialities. Nurses work closely with Physical Therapists, Occupational Therapists, Speech Language Pathologists, Home Health Aides and a Medical Social Worker. The Home Health team works closely with the patient, family, caregivers and physicians to provide high-quality care through a tailored care plan that meets the patient's individual needs. Our goal is to improve our patients' health status and to assist with maintaining an optimal level of independent function in the home.

Nursing specialties include infusion and pediatrics. We consult with the FMH Wound Care Center about patients who have complex wounds and ostomies. We have our own billing, intake, performance improvement, Medical Records Coordinator, Supply Coordinator, Scheduling Coordinators, IS Liaison and Administrative Assistants.

Clinicians are always greeted warmly by patients and their families who are very appreciative of the care we provide in the home. Nurses and therapists enjoy the autonomy and flexibility that home health affords along with the opportunity to utilize their professional skills and expertise in a very rewarding setting.

Hospice Care

Perhaps nowhere does personally connecting with patients and their families mean more than at Hospice of Frederick County. When quantity of time is no longer possible, we focus on quality of life. We strive to treat the whole person, and view the patient and family as a single unit.

By managing symptoms, easing emotional and spiritual pain, and honoring the priorities and personal choices of our patients and their families, we assist individuals to live out their remaining days in peace, comfort and dignity while supporting those they love as well.

Care is provided
…in the patient’s own home or the home of a family member. We will also care for patients in assisted living facilities, nursing homes, hospitals, or at our Kline Hospice House. Our mission is to care for all who need us, regardless of their ability to pay.

Other Services

  • Bereavement — education support groups and workshops to anyone in Frederick County including schools and businesses
  • Camp Jamie — a special weekend camp for grieving children
  • Kline Hospice House — a home-like environment where compassionate care is provided
